Complaint Narrative: Complaint received via email, contents pasted below. "I would like to report some health concerns taking place at the tavern kitchen and bar at the corner of Dougherty ferry and big Ben. I have been working there for some time and I have noticed many things. First the hand washing sinks are always blocked and filled with something. They place the food they are preparing on the shelves with the clean dishes. No one ever washes their hands. There are people eating in the dish room dairy product are always sitting out. Pan are usually still wet when they stack and use them. Sanitizer is never checked. They soak dishes in the floor mopping bucket." Location: Establishment. Observation: Establishment appears to be closed, tables pushed to one corner, bar emptied, left card for establishment to call with operating status. Action: Will monitor status. Outcome: Complaint will be closed.

Handsink made inaccessible by employee breaking handsink with block of cheese. Cilantro being prepped in handsink. Facility not able to provide hepatitis a vaccine documentation for all employees. Rear prep area has excess accumulation of food debris. Dishmachine area has excess accumulation of debris. Extremely strong sewer odor coming from interior wall east face.
